It is the policy of Hickory Grove United Methodist Church to encourage church members and visitors to take photographs and video footage at church events to be used in publications including but not limited to our website, Facebook, brochures, slideshow presentations, posters, newsletters, and other promotional materials. If you do not want to have your photograph taken, please inform anyone you notice taking photographs at Hickory Grove events that you do not wish your photograph to be taken. If you choose to opt out of having your image used, please complete this form and submit it to the Church Office.
Anyone who does not sign this form and return it to the Church Office grants permission for the use of their image on church related publications.
