This form is used to receive a refund for items purchased for church activities. A separate form is required for Each Charge Account Number used. Receipts for purchases are required in order to obtain a refund. Please include who the payment is to be made to and the address to mail the refund check.
Complete each required field and submit the form. The Financial Secretary and or Treasurer will review your request and write a check to refund you the appropriate amount.
